Providence is calling a Emergency Department Outreach Specialist (1.0 FTE, Days) to Providence Seaside Hospital in Seaside , OR. This position is full-time and will work Tuesday-Saturday, 9am-5:30pm.

We are seeking a Emergency Department Outreach Specialist that will work with a vulnerable population that has a high utilization of the Emergency Department. The ED Outreach Specialist works in conjunction with patients, team members, hospital staff and community partners to develop and implement strategies that improve the health of individuals who are identified with high utilization of emergency department services. Organizes, collects, reviews and reports health and social information through member home visits and phone outreach, while demonstrating multicultural sensitivity and effective communication skills.
In this position you will have the following responsibilities:
Conduct home and site visits to assess needs, match individuals to identified/needed services, and empowers individuals to access community resources through referrals, education, and outreach
Identify patient risk of harm to self and/or others and implements effective risk reduction, including referring patient to appropriate crisis services or another provider
Collaborate with the individual's healthcare team to recognize potentially serious problems, thus preventing poor health and social outcomes for individuals and communities
Rapidly establishes rapport and alliance with clients
Remove barrier(s) to establishing rapport and alliance, and adopts modifications in evaluation and interventions
Maintain appropriate therapeutic boundaries with patients, family/support, and others involved
Required qualifications for this position includes:
Bachelor's Degree in Social Work, Social Sciences, or human services field preferred
American Heart Association (AHA) Basic Life Support for Health Care Provider completion, or obtained within one month of hire
Valid drivers license & reliable transportation
Two (2) years of Experience working with diverse vulnerable populations, community and/or health care organization experience
Knowledge of State social service agencies and community resources
Knowledge of health systems, health education, motivational strategies, and an empathetic manner working with the underserved
Preferred qualifications for this position include:
- Direct care experience in a residential and/or outreach position serving vulnerable populations
