Providence Health & Services Social Worker - Comprehensive Pain Management in Portland, Oregon

Providence is calling a Social Worker to our Comprehensive Pain Management team based out of the Providence Professional Plaza in Portland, OR.

This opportunity is to join our new multidisciplinary Comprehensive Pain Management team that offers an outpatient 12 week program that will include education, behavior modification, group and individual encounters, Physical Therapy, yoga and acupuncture.

With a patient centered lens, the social worker will frequently collaborate with the Primary Care Medical Home team that consists of a Primary Care Provider, Case Manager, Behaviorist and Pharmacist to serve patients who are identified as needing additional skills and resources for chronic pain management. This position will be Monday through Friday during the day.

In this position, you will:

  • Independently provide comprehensive biopsychosocial/spiritual assessment, diagnosis and/or treatment/interventions/advocacy of patients and their support systems.

  • Incorporate concepts and theories associated with life span development, neurobiology, and behavioral change.

  • Collaborate with the patient and support system to develop and implement evidence-informed person-centered care plans across the care continuum in collaboration with the Inter/Multi-Disciplinary Team (IDT).

  • Perform duties in a manner that respects patient self-determination, that reduces health disparities/barriers to care, and adheres to the National Association of Social Workers (NASW) code of ethics, Standards of Practice (SOP), and mission.

Qualifications:

Required qualifications for this position include:

  • Master’s degree in Social Work from a Council of Social Work Education (CSWE) accredited school of social work, or from a school in candidacy status by the CSWE

  • Licensed Clinical Social Worker (LCSW) with the state of Oregon

  • Licensure as an Licensed Independent Social Worker (LICSW) in the state of Washington as needed based on work location(s)

  • 2 years of relevant social work experience. May include CSWA experience

  • Knowledge of community resources for mental health and addictions treatment

  • Knowledge of treatment modalities for acute and chronic pain conditions

Preferred qualifications for this position include:

  • 4 or more years of chronic pain management and/or pain clinic experience
