We are seeking a Registered Nurse (RN) Valve Clinic Coordinator - Heart Institute Administration who will be responsible for team collaboration and care delivery in a comprehensive valvular heart disease program, internally and externally. The RN Coord-Valve Clinic coordinates multiples aspects of the valve program including new patient contact, appropriate screening evaluation and maintains communication through the initial phase of treatment. The RN Coord-Valve Clinic will coordinate and work closely with the RN Coord-Valve Program to ensure seamless patient care delivery through the continuum of care.

The RN Coord-Valve Clinic position will be a liaison with referring providers and hospitals that are part of the Providence valve program, and will coordinate and ensure that all are satisfied with all components. Through collaboration with key clinical and administrative leaders at Providence, the Coordinator ensures that the managers and Providers are aware of patient progression

In this position you will have the following responsibilities:

  • Work in collaboration with the Providence Valve Center medical directors to assure patients benefit from the collective expertise of the multidisciplinary team approach to care

  • Initiate and maintain contact with the patient throughout the initial phases of treatment

  • Provide age appropriate navigation for valve patients in need of information, education and resources

  • Assess the medical history of the patient by conducting an in-depth patient intake, and provides an extensive review of patient and family care needs and with the patient, formulate and document a plan of care, or appropriate referrals based on this assessment

  • Provide extensive patient screening for appropriateness and scheduling patients for evaluation and consultation at the Providence Valve Center

  • Encourage clinical trial participation when appropriate

  • Collaborate with staff on provision of current written resources

  • Provide leadership in updating resources available in the Valve Center

  • Coordinate with patient referral and screening for cardiac clinical trials where appropriate

  • Work collaboratively with the clinical trial team

  • Be responsible for initiating appropriate oral and written communications to the appropriate person when problems arise

  • Be responsible for communicating observations to appropriate members of the interdisciplinary health care team

  • Be the key member in the weekly multidisciplinary valve center meetings and assists with coordination of patients for discussion at the weekly meeting

  • Participate in data collection and evaluation activities

  • Consistently displays positive and professional demeanor when communicating with patients, physicians and co-workers

Qualifications:

Required qualifications for this position includes:

  • Bachelor’s Degree in Nursing: Graduate from an accredited school

  • Current encumbered Registered Nurse (RN) state license

  • Current and valid Oregon State drivers license

  • Five (5) years of Clinical experience in acute care, cardiac ambulatory and/or cardiac interventions

  • Knowledge of systems, including coordination of care across the healthcare continuum

  • Knowledge of current treatments and guidelines for cardiac surgical and interventional care and the most current approaches in managing care delivery

  • Excellent verbal and written communication skills

  • Excellent networking, public relations, presentation and organizational skills

  • Skilled in collaboration, consultation, conflict resolution, facilitation and team building

  • Skilled in windows based computer technology and software, with ability to write appropriate detailed notes and projects with a work plan, action plan, and outcomes

  • Able to understand, anticipate and prioritize multiple job responsibilities

  • Able to remain flexible in response to change in work demands, variety of projects and varying time lines

  • Able to work independently but seek consultation when appropriate

  • Able to work with multiple departments, hospitals and their leaders

  • Expert interpersonal communication skills to facilitate the team process and communicate with clinical and administrative contacts both internal and external

  • Demonstrate teamwork through participation with multidisciplinary teams

  • Collaboration in the development and knowledge regarding a growing program

Preferred qualifications for this position include:

  • Master’s Degree in Nursing
