Providence is calling an RN Phone Triage, Advice - Women's Clinic East Portland (1.0 FTE, Days) to Providence 545 Building in Portland, OR

We are looking for an RN Phone Triage, Advice - Women's Clinic East Portland who will apply the knowledge, skills and experience in the provision of care via the telephone. This includes delivery of care in the form of triage, advice, referral, and patient education in the ambulatory care setting. The job requires an enhanced level of expertise in the assessment of patient needs, determination of level of care, and implementation of plan appropriate to the situation. The triage provider is expected to work closely with physicians and all other members of the health system staff, requiring excellent communication, teaching, and negotiating skills with a high expectation of total confidentiality when working with sensitive information. Position may coordinate and manage clinical activities. This can include workflow in the area by distributing workload assignments, based on patient schedules and phone demands.

In this position, you will:

  • Staffs the telephone triage and advice station. Through the nursing process, approved guidelines and established resources, collects specific data and records information in the EMR. Assesses patient's condition, provides accurate, consistent information; formulates and recommends a plan of care, validates the care plan with the patient for completeness and accuracy. Provides timely triage and assessment of patients using established guidelines and ensures appropriate documentation of same.

  • Works collaboratively with physicians and other team members as required to develop and implement the plan of care. Communicates assessment and plan to appropriate team member in a timely fashion. The assessment includes a determination of patient's needs for treatment services, the urgency of that need, and the level of care based on acuity that would be most appropriate. Demonstrates excellent communication skills, superior assessment skills, and utilizes the nursing process and nursing diagnosis in a professional manner with each telephone encounter. Provides non-judgmental advice/implements or reinforces plan of care, signs off EMR, based on guidelines and available resources.

  • Functions independently and demonstrates leadership in decision-making, communication, and problem solving techniques to resolve patient's questions and concerns. Demonstrates skill in teaching and health awareness education.

  • Coordinates / initiates the referral to the appropriate provider as needed. Works closely with clinic manager, physicians and other staff to complete the referral

Qualifications:

Required qualifications for this position include:

  • Graduate from accredited school of Nursing

  • Associate's Degree in Nursing

  • 5 years experience in health care

  • Current Oregon state license as a registered nurse in good standing

  • BLS certification

Preferred qualifications for this position include:

  • Bachelor's Degree in Nursing or related field

  • 1 year experience in clinical triage/advice

  • 1 year experience in ambulatory healthcare setting

  • Bilingual in Spanish
