Providence is calling a RN - Hospice East Portland (1.0 FTE, Days) to Providence Office Park Portland in Portland, OR.

We are seeking a RN - Hospice East Portland that will complete medical requirements while coordinating a whole person plan of care. Be accountable for direct and indirect quality care for patients of all ages. Care is according to the individualized needs of the patient, following the nursing process and in accordance with the Nurse Practice Act. Position accepts professional development as an integral part of nursing practice. Coordinate patient care activities with other members of the health care team in a collegial relationship. Uphold the standards of confidentiality, attendance and punctuality.
In this position you will have the following responsibilities:
Complete assessments for terminal illness and gathers essential information needed for plan of care
Update assessments in collaboration with the patient, family and/or caregiver identifying changes in patient conditions and progress towards desired patient outcomes
Provide coordination of care
Maintain patient clinical records in accordance with regulatory standards
Perform procedures and treatments according to Providence Hospice professional standards, policies and procedures
Counsel patients and caregivers on patient care in accordance with the patient and family wishes, beliefs and values
Provide written instructions and supervision for the hospice aide per Providence policies and procedures
Delegate tasks to patient caregivers following regulatory requirements and Providence policies and procedures
Manage an assigned patient caseload
Understand current knowledge base of payor and department reimbursement requirements and processes
Work assigned hours including rotating through weekend shifts and after hours call
Participate in the Quality Assessment Performance Improvement Plans
Maintain knowledge base for End Of Life care: pain and symptom management, bereavement, ethics, hospice philosophy, and community resources
Document time and activities in time log/transaction log in accordance with Providence policies and procedures and state and federal laws
Qualifications:
Required qualifications for this position includes:
Unencumbered Oregon nursing license
Current Basic Life Support (BLS) for Healthcare Provider -OR- Advanced Cardiac Life Support (ACLS) within 30 days of hire
Valid driver's license and reliable transportation
Driving record consistent with GOP Safe Driving Policy 330 Incident Point Matrix at time of hire
Two (2) years of medical/surgical experience in an acute care setting, -OR- Combination of two (2) years of medical/surgical, long term care or home health or hospice experience
Experience using electronic medical record (EMR) software
