We are seeking a RN Home Health Access - Home Services Home Intake that will arrange prescribed services with the physician, payor, patient, caregiver, and provider to ensure that services are provided in a timely and coordinated fashion. This position also processes triage calls from existing patients and caregivers, addressing clinical issues and forwarding information to individual team supervisors
In this position you will have the following responsibilities:
Represent the mission of the Providence Health System in all aspects of Home Health referral coordination functions and nursing triage
Coordinate Home Health services provided directly by Providence Home Health Services
Take patient referral information by phone or via fax and obtains necessary information either on-line or by other means to complete the referrals
Work with the HH Access Team to process referral including entering documentation into the electronic medical record
Plan and organizes home services for patient discharges in the local and regional service areas
Coordinate with the Home Health Scheduling team to determine the start of care date for home services
Coordinate with other Home Services departments in order to process referrals
Triage calls from existing patients/caregivers, addresses clinical issues and forwards information to individual team supervisors and clinicians
Demonstrate advancement of professional nursing practice through continuing education, professional organizations and committee activities
Activate the Home Services Disaster/Triage Plan as per protocol including after hours, weekends and holidays
Maintain positive working relationships with home care providers, payors, case managers, physicians and their office staff, hospital and extended care facility care managers and discharge planners, and Providence Home Services departments
Maintain information on current Providence Home Services payor contracts and subcontract relationships
Access Intranet contract information and tools
Attend staff meetings and participates in department quality improvement initiatives and in-service education
Stay current with community health issues and regulations
Work assigned hours including rotations through weekend shifts
Required qualifications for this position includes:
Current unencumbered Oregon RN license
Experience using electronic medical record software
Preferred qualifications for this position includes:
Bachelor's Degree in Nursing
Nursing Triage Call Center experience
General knowledge of state and federal regulations and Joint Commission standards for home care services
An understanding of how health care delivery system works and the interrelationships among patients, physicians, hospitals, home care providers, community agencies, suppliers and third party payors
