We are seeking a RN ALS Program Coordinator - Neurological Specialties East that will responsible for monitoring medical, physical, and psychosocial care needs of patients receiving care from the Providence Amyotrophic Lateral Sclerosis (ALS) Center. The RN provides direct patient care and case coordination from diagnosis to end of life. This position coordinates the care of the patient through assessment, planning, implementation and evaluation of appropriate services and resources to maximize optimum outcomes. Works collaboratively with the multiple disciplines, both internal and external to the organization, in effecting quality and cost-effective management of the patient's care along the continuum of chronic neurologic disease.
In this position you will have the following responsibilities:
Use the nursing process to assess, diagnosis, plan, implement, and evaluate for patients with neurological illnesses
Collaborate with the Neurologist and sub-specialty team to facilitate and coordinate services, supplies, and equipment needed by patient and family. This includes involving family, as needed, in team meetings, planning and coordination of care, education, and identification of resources needed to develop realistic expectations of the chronic neurologic care
Provide appropriate short-term counseling for patients and families whose psychosocial situation demonstrates the need for intervention
Triage telephone calls and messages, systematically investigating and assessing patient needs determining whether and when patients require appointments, as appropriate, considering age-appropriateness
Assist patient and families to identify needs for the hospital stay, discharge, and follow-up care
Order, refill, and dispense prescriptions, and complete paperwork as directed by physicians
Identify and assist physician and other personnel in stabilizing, treating and/or triaging patients who may need a higher level of care
Required qualifications for this position includes:
Bachelor's Degree Nursing; from an accredited RN School of Nursing
Two (2) years in Nursing experience working with patients who have neurological conditions or ambulatory care, home health, acute care and/or case management
Background in telephone triage, outpatient clinic, ambulatory care, home health, acute care, hospice, and/or case management
Experience with an electronic medical record system
Proficiency in keyboarding, Microsoft Outlook, Microsoft Word
Knowledge of ALS management
Current Oregon RN license
Current AHA-BLS certification
Preferred qualifications for this position includes:
