Providence is calling a Pharmacy Technician - Medication History (1.0 FTE, Evenings) to Providence Portland Medical Center in Portland , OR. This position's shift will be 8 shifts on followed by 6 shifts off. Shifts will be 10-hour shifts and work from 1:30pm to 12:00am. You will get an evening differential added to your pay!

We are seeking a Pharmacy Technician - Medication History that will conduct patient, family or caregiver interviews to obtain a list of medications a patient was taking prior to admission. If needed, the MHtech may utilize other data sources to further investigate and complete an accurate medication list. The medication list compiled by the MHtech will be entered into the hospital's computerized medication reconciliation program and verified by a registered pharmacist. On occasion, may perform pharmacy technician job duties as assigned.
In this position you will have the following responsibilities:
Perform medication history assessments for new admissions to the hospital, providing complete information regarding each medication identified (drug, dose, route, frequency, indication for PRN, and last dose taken-date/time)
Verify patient allergy information and ensure complete documentation (substance, allergy category, reaction description)
Coordinate medication history assessment
Document the medication history and allergy information in the electronic medical record accurately and in a timely manner
Participate in performance improvement activities
Participate in the training of Pharmacy Technicians, Externs, and Pharmacy Students regarding home medication interviews and documentation
In addition to these specialized responsibilities, the Medication History Technician will also be able to function as a standard pharmacy technician
Assist the pharmacists in preparing and distributing drugs and maintaining drug inventory
Knowledge and application of hospital wide compliance policies as well as specific compliance issues for area of accountability
Required qualifications for this position includes:
- Current Oregon Board of Pharmacy Technician license
Preferred qualifications for this position includes:
Graduate of a Pharmacy Technician program
Experience with current pharmacy information systems (i.e. EPIC, Pyxis applications).
Two (2) years of experience as a certified Pharmacy Technician in retail or inpatient setting
Experience working directly with patients
