Occupational Therapist - Laurelhurst Day Center

This position is responsible for OT evaluations, and development and implementation of the OT plan of care in accordance with PACE policies. The OT is a member of the Interdisciplinary Team and responsible to communicate and coordinate care with the team as well as multiple outside providers, caregivers and family members. The OT functions independently, working in multiple settings including the PACE center, participant homes, Adult Family Homes, Assistive Living Facilities and nursing homes. The OT assumes responsibility for participants treated by the OTA and OT Aide

Qualifications:

Required qualifications for this position includes:

  • Bachelor's Degree in Occupational Therapy

  • Valid Driver's License

  • Current auto liability insurance

  • Current licensure as an Occupational Therapist by the state of Oregon required.

  • American Heart Association Basic Life Support (AHA-BLS) for Health Care Provider certification required at time of hire

Preferred qualifications for this position includes:

  • Master's Degree in Occupational Therapy or OTD (Occupational Therapy Doctorage)

  • One (1) year experience working as an occupational therapist in a geriatric setting.
