Providence is calling a Coder Home Services - Hospice Access (0.8 FTE, Days Full Time) to Home Service Building in Portland , OR

We are seeking a Associate Coder Home Services - Hospice Access that will review and code diagnoses in the electronic medical record (EMR) using ICD-10 coding principles. Ensures diagnosis codes meet optimum specificity standards, are sequenced in a manner that supports the care provided to the patient, and conforms to coding conventions and Centers for Medicare & Medicaid (CMS) and other regulatory requirements. Completes this work in a manner that supports Health Information Management data integrity standards.

In this position you will have the following responsibilities:

  • Review and interpret clinical documentation to identify all diagnoses

  • Apply ICD-10 CM diagnosis codes to Home & Community Care records for clinical care, billing, and reporting purposes

  • Sequence principal and associated diagnoses in accordance with coding conventions and ensures diagnoses are coded to the highest level of specificity

  • Escalate records where the primary diagnosis does not meet coding criteria

  • Participate in department/team meetings

Qualifications:

Required qualifications for this position includes:

  • Obtain one of the following coding certifications within 12 months of hire:

  • AHIMA Certified Coding Associate (CCA) coding credential; -OR-

  • HCS-D coding credential; -OR-

  • HCS-H coding credential

  • Two (2) years of experience in a team setting which may include 6 months experience in a clerical or general office setting

  • Effective interpersonal skills

  • Assume personal responsibility for actions

  • Able to work within tight timeframes and meet strict deadlines

  • Demonstrate a high commitment to quality

  • Seek to acquire knowledge in area of specialty

  • Proactively approaches responsibilities

Preferred qualifications for this position includes:

  • Coursework/Training: Anatomy/Physiology and Medical Terminology

  • Experience using an electronic medical record (EMR), diagnosis coding software, or related application

  • Medical terminology, anatomy and physiology, and human disease process knowledge
