Providence Health & Services LPN - Home Health West Portland in Portland, Oregon

Providence is calling a Licensed Practical Nurse - Home Health West Portland (1.0 FTE, Days) to Cedar Hills Home Services in Portland, OR.

We are seeking for LPN who will LPN provides basic patient care and specific delegated duties that support the delivery of quality patient care, under the supervision of an RN and physician orders within a chemical dependency environment. The LPN must be competent to interact and gather data from patients of varying ages, including: adult, geriatric and specialized populations.

In this position, you will have the following responsibilities:

  • Provides clinically appropriate nursing care according to department policies, protocols, and current OARs.

  • Observes, documents and promptly reports patient responses or changing conditions to RN or appropriate healthcare provider.

  • Conducts patient education and instruction, considering age-appropriateness, based on department protocols.

  • Triage work to scope of practice.

  • Orders supplies for department per protocol. Maintains an adequate inventory of medical supplies.

  • Attends meetings and in-services per department policy.

  • Assists RN and coworkers with duties as needed.

  • Performs general nursing care to patients, considering age-appropriateness. Administers prescribed medications and treatments in accordance with nursing standards and physician orders.

  • Maintains a current knowledge and competency in behavioral health evidence based nursing practice, treatment of substance abuse and mental health by ongoing education in specialized field of practice.

Qualifications:

Required qualifications for this position include:

  • Graduate of an accredited LPN/LVN school of nursing.

  • 3 years experience in behavioral health nursing.

  • 1 year experience in acute care setting.

  • 3 years experience in substance abuse treatment.

  • Fingerprinting may be required
