Providence Health & Services Licensed Practical Nurse - ElderPlace in Portland, Oregon
Providence is calling a Licensed Practical Nurse - Irvington Nursing Services to Providence ElderPlace Belmont in Portland, OR.
In this position, you will:
Provide quality nursing care in a clinic or community setting, under the direction of a Registered Nurse (RN).
Perform all duties within the standards and scope of practice for the Licensed Practical Nurse (LPN).
Function within the Interdisciplinary Team (IDT) to formulate and evaluate the Participant’s Plan of Care (POC).
Complete focused assessments, by collecting both subjective and objective data.
Assess educational needs and develop trainings for participants, families and caregivers.
Identify areas of need or potential change in a participant’s status.
Perform clinical tests, treatments, therapies, medication administration, health teaching and counseling, as directed by the Primary Care Physician (PCP) and RN.
Required qualifications for this position include:
Current unencumbered Licensed Practical Nurse (LPN) license by the Oregon State Board of Nursing
Current Basic Life Support (BLS) for Healthcare Providers
In compliance with company policy and state regulations, a Department of Human Services (DHS) criminal background check is required for this position; fingerprinting may also be required
Preferred qualifications for this position include:
1 year of experience with a frail, elderly population
Experience in a clinic or clinic-like setting
Home Health experience
Long term care experience
