We are seeking a Infusion Clinical Nursing Coordinator (RN) - Home Services Infusion Nursing that will coordinate the nursing services of Providence Home Infusion. This includes process development, patient and staff scheduling, coordination with pharmacy and oversight of daily work flow for delivery of nursing care.
In this position you will have the following responsibilities:
Coordinate Initial Plan of Care:
Facilitates communication, coordination and problem-solving with physician, patients, families and staff to develop initial infusion plan of care
Works with multiple disciplines from pharmacy, care management and outside agencies to establish an appropriate and cost-effective plan
Provide ongoing Care Coordination and Scheduling:
Be responsible for overseeing the daily coordination of workflow and scheduling of nursing care in the delivery of services
Work with primary RNs and patients to assist in managing scheduling issues
Serve as clinical expert nursing consultant to hospital liaisons, discharge coordinators and community agencies to assure continuity of care for patients transitioning between providers
Maintain a high degree of visibility with the staff, patients, referral sources and supporting institutions.
Participate in the delivery of direct patient care activities as necessary
Maintain in-depth nursing knowledge and clinical expertise in the specialty of Infusion
Assure nursing responsibilities are met for authorization from payors. Works with support personnel to facilitate admission process
Work in cooperation with the Authorization staff to assure appropriate information is available regarding authorization for services
Required qualifications for this position includes:
Current unencumbered nursing license for the state of Oregon
A Washington Nursing License must be attained no later than 30 days from start date
Current BLS (American Association Basic Life Support for Health Care Provider) required within 30 days of start date
Three (3) years of Home Infusion nursing experience
Preferred qualifications for this position include:
- Bachelor's Degree in Nursing
