We are seeking a Liaison Home Medical Equipment - Home Service Liasons that will coordinate home medical equipment (HME) services for patients being discharged from a designated hospital. The Liaison work at an assigned hospital with hospital-based Acute Care Managers (ACM), Patient Care Coordinators (PCC), discharge planners, and Medical Social Workers (MSW). The Liaison collect and prepare all necessary documentation required for a complete order; communicate with Access Services to complete the coordination of the order. The position act as a resource and educate hospital staff regarding the services offered by HME and contribute to the patient care plan as it relates to the discharge plan. The Liaison provide information to ensure a smooth transition for patients and their families following hospitalization.
In this position you will have the following responsibilities:
Coordinate HME services and process durable medical equipment orders for patients discharging from the hospital. This includes all DME, respiratory equipment, oxygen and wound vacs for patients utilizing internal and external vendors
Meet with patient and or caregivers to explain HME services being provided and obtain necessary documentation to provide care
Ensure that services ordered are appropriate, e.g. equipment ordered is appropriate based on clinical criteria and patient diagnosis and that the required physician prescription(s) are present in the patients chart
Obtain accurate charting and MD orders to ensure appropriate insurance coverage
Train patient or caregivers on the operation and use of home medical equipment devices for the purpose of using those devices to transfer from the hospital to the patients residence
Coordinate teach times for advanced pieces of equipment
Enter patient data and appropriate documentation into the appropriate Home Services databases
Responsible for providing all necessary documentation to external vendors to ensure patients receive all DME ordered for discharge from hospital
Coordinate the delivery of equipment to the patients residence with HME Access Services and HME Dispatch as required
Also responsible for arranging and coordinating deliveries for outside vendors to avoid discharge delays
Facilitate integrated DME services for patients discharging from hospital for PHS
Serve as a resource for physicians, specialists and care management regarding documentation, testing and DME covered diagnoses for patients discharging from the hospital
Required qualifications for this position includes:
Two (2) years of experience in the home medical equipment field
Two (2) years of experience in HME customer service or HME insurance verification
Ability to simultaneously handle multiple priorities
Preferred qualifications for this position include:
Bachelor's Degree -OR - equivalent educ/experience
Familiar with the setup and operation of standard HME items like walkers, canes, crutches, commodes, basic wheelchairs
One (1) year experience with CMS regulations regarding DME
