Providence Executive Director, Care Management in Portland, Oregon


The Executive Director, Care Management is responsible for strategic leadership and management of initiatives to improve population outcomes related to clinical quality and the total cost of care across inpatient, PMG, Home and Community Services and PHP settings. Approximately 20% of the work of this role is directed toward inpatient care management and 80% toward PMG and Clinical Services and integration across the continuum. The Executive Director, Care Management is responsible for developing data-driven, strategic, operational and implementation plans for multiple initiatives to achieve population health management objectives in service to Providence's core organizational strategy of "Creating healthier communities, together". This position will have direct reporting relationships or coordinate oversight of inpatient care management functions in all hospitals in the Oregon Region. This person in this position will work in partnership with PMG, Strategic Planning Services, Home and Community Services and Providence Health Plan leadership to build needed population management infrastructure to ensure alignment of strategies and activities and to improve/implement processes, care delivery models and interventions to support lowering the total cost of care through a coordinated and connected care experience for Providence care givers and patients/members.

Essential Functions

  • Improve Care Management and care coordination across the continuum via direct and indirect reporting, as well as coaching/leadership via the Care Management Integration Council.

  • Assist PMG and Clinical Services operations to become more efficient and effective in achieving total cost of care objectives, in alignment with clinical quality and access objectives (i.e., Acute Care Transformation). Provide guidance for the refinement and evolution of medical home/medical neighborhood model across the region.

  • Serve as liaison for statewide care management-focused initiatives and for project related activity within Providence Health Systems and within the Oregon Region, in the context of region-wide Affordability work.

  • Provide on-going communication to key stakeholders to ensure the required support is provided for initiatives to be successful and issues identified have been addressed.

  • In partnership with the Chief Nursing Officer and PMG CMO, foster and oversee innovation that improves quality and affordability.

  • In partnership with the Chief Nursing Officer and PMG CMO, engage physicians in population health improvement and support development of a culture of provider led innovation.

  • Identify and test potential best practice models, processes, tools, and interventions to meet Triple Aim objectives (i.e., BEST).

  • Supervise and/or direct the work of various project teams and direct reports to ensure the sum of the work improves outcomes and the health of the communities we service (i.e., oversight of the Health Resilience contract with CareOregon).

  • Work to create the infrastructure required to further develop population health competencies across Providence business units and risk pools.

  • Influence information technology solutions at Regional and System level to support cross-continuum care management and coordination (i.e., Epic functionality, Emergency Department Information Exchange system).

  • Advance capabilities to identify high risk and rising risk patients/members who may need assistance to improve their health and utilization patterns (i.e., predicting risk of readmission).


Minimum required qualifications:

  • Master's degree in Health and/or Hospital Administration, Business, or related field.

  • 10 or more years of management experience in a clinical setting.

  • Experience in progressively responsible roles with demonstrated leadership experience and strong team management skills.

  • Experience with physicians.
