We are seeking a Clinical Pharmacist - Credena Health - Home Services Specialty Pharmacy that will be responsible for coordination of medication related services as it pertains to specialty pharmacy. Responsibilities include promotion of evidence-based, safe, effective drug therapy, standardization of medication practices across the continuum, and coordination with drug procurement, inventory management and billing processes.

Credena Health pharmacists interpret, dispense and monitor all physician prescriptions. Pharmacists provide the best possible drug therapy to Credena Health patients under the highest of professional standards.

The pharmacist works closely with nurses, physicians and allied healthcare professionals to assure that patients receive quality pharmaceutical care, delivered in a safe and efficient manner.

The Credena Health pharmacist performs all duties in a manner that promotes a team concept and reflects the Providence mission, core values, service excellence and behavior standards.

In this position you will have the following responsibilities:

  • Interpret orders and prescriptions for all medications to be provided to patients

  • Assure that the appropriate dose and route of administration have been ordered and use pharmacokinetic and other dosing regimens to accurately suggest dosing for patients

  • Monitor patient’s clinical status in relation to drug therapy through communication with the nurse clinician, patient, and physician and review laboratory results to detect drug effectiveness, side effects, toxicity and/or adverse reactions

  • Document patient consultation appropriately in the electronic medical record outlining issues regarding the patient and adherence to medication regimen as needed

  • Contribute to smooth transitions of care for the patient between health care settings in collaboration with the physician, floor nurse, clinical nurse, home health nurse, other pharmacist and other professionals

  • Ensure accurate and timely processing of orders in the pharmacy information system addressing all aspects needed to complete accurate billing

  • Complete checking of technician’s work and the dispensing of medication and ancillary supplies required for the proper administration of the product by the specified route of administration

  • Precept pharmacy resident and intern rotations

  • Review patient records and orders regarding drug therapy, recommends and initiates changes that are consistent with the patient’s status. Is successful in improving patient outcome through monitoring and intervention

  • Supervise pharmacy staff to provide an environment conducive to quality pharmacy practice and medication safety

  • Ensure that the patient knows the indications, side effects, drug interactions, and storage of the medication

  • Serve as a drug information resource for physicians, nurses, other health care professionals, and patients

  • Ensure that Credena Health complies with federal and state pharmacy laws as well as meets the standards set forth by The Joint Commission and other regulatory agencies

  • Maintain professional proficiency by attending workshops, seminars and classes to remain current with new developments in drug therapy

  • Provide educational in-services to other pharmacists and pharmacy technicians on the various aspects of drug therapy as requested or needed

  • Train new staff in procedures and departmental policies

  • Maintain awareness of and complies with Quality initiatives and programs

Qualifications:

Required qualifications for this position includes:

  • Currently licensed as a registered pharmacist in good standing by the Oregon Board of Pharmacy or the ability to become licensed prior to employment

  • Comprehensive clinical pharmacy knowledge and practice experience base with emphasis in the field of specialty pharmacy

  • Able to evaluate and synthesize data and apply evidence-based medicine principles to practice

  • Demonstrated ability to effectively establish and maintain working relationships with peers and constituents at all levels of the organization

  • Excellent interpersonal communication skills and the ability to communicate effectively in verbal and written form with the health care team and with patients

  • Minimum of two (2) years experience in hospital IV pharmacy practice
