Providence Clinical Pharmacist - Anticoagulation Specialist in Portland, Oregon

Clinical Pharmacist - Anticoagulation Specialist. This position will work M-F from 8:30am to 5pm (5 8-hour shifts). We are looking for someone to work out of Providence St. Vincent Medical Center in Portland, OR. There is potential to float to Providence Willamette Falls Medical Center and Providence Portland Medical Center if there is need.

The Clinical Pharmacy Specialist will collaborate with providers and caregivers to provide coordinated, comprehensive, high-quality care to patients referred to the PH&S Anticoagulation and Pharmacotherapy Services for focused drug therapy management of acute and chronic disorders. The Clinical Pharmacy Specialist is responsible for providing collaborative drug therapy management (CDTM) services in clinic, telephonically, by e-visits, and possibly telehealth. Upon referral, the Clinical Pharmacy Specialist will assess the patient, initiate and adjust medications, screen for and minimize drug interactions and adverse effects, provide disease/medication education, and explore opportunities to simplify &/or decrease the cost of the medication regimen in collaboration with the referring provider (PCP)/prescriber, patient/their caregiver and other Providence team members. They will also educate team members to continually advance the service level of practice.

Qualifications:

Required qualifications:

  • Graduate from an accredited school of pharmacy

  • Post-graduate year residency (PGY1) -OR- three (3) years experience in area of specialization (ambulatory care with direct, collaborative patient care) in lieu of residency

  • Oregon State licensure as a Pharmacist or ability to obtain license prior to employment

  • Board Certified Pharmacotherapy Specialist (BCPS) or Board Certified Ambulatory Care Pharmacist (BCACP), or other Board of Pharmacy Specialties certification in area of specialization prior to or within two (2) years of hire as appropriate.

  • Basic Life Support (BLS) within six (6) months of hire

  • Hands-on experience in relevant patient assessment, medication adjustment, monitoring, and all other aspects of patient care

  • Experience in writing, including educational pieces for patients and healthcare providers

  • Experience precepting students and/or residents

Preferred qualifications:

  • PGY-2 residency in ambulatory care
