Providence Health & Services Clinic Navigator - Elderplace Laurelhurst in Portland, Oregon
Providence is calling a Clinic Navigator (1.0 FTE, Days) to Providence Elderplace Laurelhurst in Portland, OR.
We are seeking a Clinic Navigator who will be responsible for the flow of patients seen in the PACE centers. They coordinate room set-up and clean-up. Assists with obtaining vital signs, capillary blood glucose (CBG) results and other information needed for patient visits, completes labs requests, assists with the personal care of patients, and documents care given.
In this position, you will:
Coordinate participant check-in, rooming and general support related to clinic appointments for PCP, nurses, and medical specialists.
Perform various clinic skills such as: vital signs, weights, pulse oximeter, EKGs and point of care testing as necessary.
Document vital signs and weights in the electronic health record (EPIC) prior to the PCP visit and completes other documentation, such as observations by the end of the shift.
Complete lab requests and specimen collections, including phlebotomy, as ordered.
Maintain infection control practices according to PEP policy.
Coordinate room set-up and clean-up including ordering, stocking, and tracking clinic supplies.
Provide personal care and assistance based on participant needs.
Provide back-up for the Clinic Assist in completing EPIC scheduling, printing out the After Visit Summary (AVS) and scanning as requested by clinicians.
Required qualifications for this position include:
High school graduate or equivalent
Current Basic Life Support (BLS) certification
Current Standard First Aid
Medical terminology training or equivalent education/experience
2 years of experience working with, or caring for, a frail or elderly population in a professional setting
In Compliance with the Department of Human Services (DHS), criminal background check is required for this position, based on assigned duties. Fingerprinting also may be required
Preferred qualifications for this position include:
- Certified Nurse Assistant (CNA) certification
