Providence Health & Services Aide-Personal Care in Portland, Oregon

Providence is calling a Personal Care Aide - Gresham Day Center (1.0 FTE, Days) to Providence Elderplace Oregon in

Portland, OR.

We are seeking a Personal Care Aide - Gresham Day Center who will work under the direction of a Registered Nurse,

provide personal care and assistance to ElderPlace participants. The position creates/maintains a physical and social

environment consistent with residence care needs.

In this position you will have the following responsibilities:

  • Provide or assist participants with activities of daily living skills, including but not limited to feeding, bathing, toileting,

grooming and ambulation

  • Assist geriatric participants with general or selected activities as assigned

  • Assist with or perform homemaking duties for participants which may include: shopping, laundry, housecleaning, meal

preparation, errands and assisting with medications as directed

  • Provide treatment and/or procedures under the direction of a Registered Nurse

  • Perform and document participant weight, temperature, pulse, respiration, and blood pressure and other observations in

appropriate medical records

  • Report all pertinent observations concerning participants to appropriate medical staff or supervisor

  • Participate in participant initial and review evaluations / service plans

  • Assist with restorative and maintenance programs for participants in conjunction with OT and PT departments

  • Assist with preparation and distribution of meals and snacks

  • Maintain general order, infection control, and cleanliness of the center/home site, equipment and supplies

  • Participate in emergency and fire drill exercises

  • Recognize emergency situations and is familiar with Disaster policy and procedures

  • Answer telephone, takes and relays messages as directed

  • Serve as EP site liaison related to clinic appointments

  • Communicate effectively across site work groups / areas

The Personal Care Aide under the direction of a Registered Nurse provides personal care and assistance to ElderPlace participants. The position creates/maintains a physical and social environment consistent with residence care needs.

  2. Maintaining a positive attitude despite constant disruption and interruptions.

Patient Population Served: Adult, Geriatric

Consequently, the employee must be competent in the interpretation of a patient's self-report or behavior, and this information must be interpreted with an understanding of the cognitive, physical, emotional/psychosocial and chronological maturation process. The treatment of patients should be individualized and should reflect an understanding of the developmental needs and range of treatment needs for each patient.

ESSENTIAL FUNCTIONS

The caregiver must be sufficiently fluent in the English language to satisfactorily perform the essential functions of the position. The degree of fluency required will vary depending upon the nature of the position.

Qualifications:

Required qualifications this position include:

Education:

License/Certification:

o CPR and Standard First Aid.

o AHA BLS CPR must be obtained within six months of hire.

o Current caregivers have until 2019 to obtain AHA certification, which then must be maintained.

o In compliance with company policy and state regulations, a Department of Human Services (DHS) criminal background check may be required for this position based upon the location of the position.

o Fingerprinting may be required.

Experience:

1 year Housing Only: Recent experience, as a caregiver, with frail or elderly population.

Preferred qualifications for this position include:

Education:

License/Certification:
