Providence Health & Services RN Manager Clinical Outreach Services in Oregon
Providence St. Joseph Health is calling a RN Manager Clinical Outreach Services based out of our location in Portland, OR. This role is currently remote, however it may change to a hybrid role in the future. Qualified candidates MUST have or be elegible for dual RN licensensure.
We are seeking a RN Manager Clinical Outreach Services who will be responsible for effective development, implementation, and evaluation of the clinical outreach strategy and tactics aimed at improving the quality of care thru patient engagement and proactive care gap closure. This position has shared accountability for the education, clinical practice and safety protocols used by the Clinical Outreach Service caregivers. It oversees Clinical Outreach Care Coordinators caregivers providing daily leadership with clinical oversight over operations and patient or provider related issues and concerns. The manager is responsible for leading and delegating specific clinical outreach workflows using various patient engagement skills and tools. The RN Clinical Outreach Manager is the point person in managing questions and issues as they arise from clinic caregivers, providers, and patients in all regions receiving services from the team. In this role, the RN Clinical Outreach Manager assures practice is in accordance with State, National, and Medical Group evolving regulatory or governance criteria or professional practice of the care coordinators. This position is a Clinical Outreach Service liaison between Physician Enterprise providers, clinical and operational leaders, and leadership governance groups overseeing clinical quality improvement. Performs all duties in a manner that promotes team concept and reflects the Sisters of Providence mission and philosophy.
In this position you will have the following responsibilities:
Manage the daily clinical operations and functions of the Physician Enterprise Clinical Outreach Service. Recruits, hires, manages caregivers. This role is accountable for establishing goals and measures of success for the area of responsibility, which are aligned with the departmental and line of business strategic goals and deliverables.
Provide regular education and communication about clinical and operational standards of performance; applies coaching and motivational interviewing skills. Leads performance evaluation process fostering an environment of quality improvement by setting individualized caregiver goals with clear expectations.
Lead clinical competency program, work safety, scope of practice compliance for the department identifying educational opportunities for caregiver performance improvement and development.
Manage assessment and distribution of duties to ensure a balanced workload across the team and timely completion of assignments.
Engage with leaders, caregivers and various teams across the organization to identify, design, adapt, and adopt solutions aligned with organizational priorities for quality, patient and caregiver experience improvements.
Communicate and collaborate with department leadership team and patient care team regarding daily operational issues.
Lead or participate in regularly scheduled meetings to distribute or acquire information, contribute ideas to improve effectiveness and efficiency of outreach processes, trainings, or other service line related collaboration.
Develop and maintain key relationships between the Clinical Outreach Service and the organization thru excellent communication and accountability of work.
Navigates thru data system(s) to research and evaluate performance, productivity or troubleshoot issues related to the team's scope of work.
Required qualifications for this position include:
Bachelor's Degree in Nursing.
3 years Experience in supervisory or leadership position.
5 years Experience of direct care.
Epic EMR experience.
Experience with electronic medical records and/or computerized billing systems.
Experience in utilizing disease registries or similar databases.
Experience in utilizing databases and spreadsheets.
Demonstrated written communication skills
Strong verbal communication and listening skills.
Develops and delivers effective presentations.
Ability to build strong customer relationships.
Effective leadership skills.
Demonstrates a high commitment to quality.
Demonstrated time management and priority setting skills.
Registered Nurse (Washington & Oregon).
Preferred qualifications for this position include:
Master's Degree inNursing / Public Health .
5 years Experience of primary care.
