Providence is calling a RN Clinical Liason- Home Health Services (0.6 FTE, Days) to Providence Benedictine Nursing Center in Mount Angel, OR.

We are seeking a RN Clinical Liason - Home Health Services who will represent all divisions of Providence Home Services (HS) at facilities and requires strong communication and interpersonal skills. The role of the Clinical Liaison is to coordinate and arrange home care services between HS and facilities for home health, home infusion, and home medical equipment associated with a clinical referral.
In this position you will have the following responsibilities:
Coordinate care for all services being provided after the Nurse Case Manager/or Discharge Planner provides the patient with freedom of choice for a home services agency and contacts the HS Liaison with an order for Home Services. Anticipate supply and equipment needs required to provide home services.
Meet with the patient to explain home services being provided and obtains necessary documentation to provide care. Assesses that services ordered are appropriate.
Document in the hospital record the plan of care for discharge with the name of the agency, phone number, start of care date and services ordered.
Enter patient data and appropriate documentation into the appropriate Home Services databases
Coordinate with HS intake and scheduling staff to determine start of care date for home services
Maintain productivity requirements in facilitating timely referrals to Home Services.
Advocate for patient/family. Follow practices that maintain professional boundaries and avoids conflict of interest. Consider patient values even when they differ from the nurse's personal values
Identify and document unusual occurrences accurately and timely. Understand the customer concern / customer grievance process. Participate in Performance Improvement activities
Required qualification for this position includes:
Current, unencumbered Oregon RN license required
Current Basic Life Support (BLS) for Provider Certification or ability to obtain in 30 days. Renewal required every 2 years
One (1) year full-time experience in home health or discharge planning
Experience in home care and in positions that required extensive customer service skills.
Preferred qualification for this position includes:
Bachelor's Degree in Nursing
One (1) year full-time experience in home infusion
