Providence Health & Services LPN - Home Health in Mount Angel, Oregon


Providence is calling a LPN - Home Health Services (1.0 FTE, Days) to Benedictine Home Health in Mount Angel, OR.

We are seeking a Licensed Practical Nurse (LPN) - Home Health Services who will provide quality care to patients in the nursing center and their homes under the direction of a Nurse Case Manager or Clinical Supervisor following the established nursing process and procedures, and in accordance with the Nurse Practice Act. Performs all duties in compliance with all applicable State and Federal regulations and in compliance with all regulatory bodies. LPN's working in Home Health require 25% to 35% of time on daily travel.

In this position you will have the following responsibilities:

  • Manage an assigned patient caseload that includes patients requiring a broad spectrum of care, who reside in a variety of environments, and may reside in a wide geographical area

  • Assist the RN in assigned duties/procedures providing high quality and safe patient care

  • Document care and maintain accurate and current EHRs according to department policy and in keeping with all state and federal regulations

  • Develop, maintain and revise office records to meet department and hospital standards, and state and federal regulations

  • Teach, supervise and counsel patients/caregivers regarding nursing care and other related issues to the home situation

  • Coordinate patient care with other members of the multi-disciplinary team as appropriate

  • Report acute medical problems, and condition changes including changes in the social condition and environment to the attending Physician and RN Case Manager per agency policy

  • Attend mandatory meetings

  • Participate in activities which maintain/enhance professional expertise and clinical skills

  • Participate in the department's rotating weekend shifts


Required qualifications for this position includes:

  • Education to meet certification, license or registration requirement

  • Current unencumbered Oregon LPN license

  • Current AHA-Basic Life Support (BLS) for Provider Certification or obtained within sixty (60) days of hire

  • Valid driver's license and a driving record consistent with Safe Driving Policy 330 Incident Point Matrix at time of hire

  • Two (2) years Patient care experience in a skilled nursing facility -OR- medical clinic

  • In Compliance with the Department of Human Services (DHS), a criminal background check is required for this position

Preferred qualifications for this position include:

  • Experience in Acute Care, Home Health -OR- Public Health nursing
