Providence is calling a Physical Therapist - Milwaukie Day Center (1.0 FTE, Days) to Providence Elderplace Oregon in Milwaukie, OR
We are seeking a Physical Therapist - Milwaukie Day Center who will be responsible for PT evaluations and development and implementation of the PT plan of care in accordance with PACE policies. The PT is a member of the Interdisciplinary Team and responsible to communicate and coordinate care with the team as well as multiple outside providers, caregivers and family members. The PT functions independently, working in multiple settings including the PACE center, participant homes, Adult Family Homes, Assistive Living Facilities and nursing homes. The PT assumes responsibility for participants treated by the PTA and PT aide.
In this position you will have the following responsibilities:
Perform thorough PT assessments evaluating physical and functional mobility status of PACE participants including but not limited to ROM, strength, balance, sensation, coordination and gait. Determine need for PT interventions, caregiver training and durable medical equipment.
Develop plan of care in accordance with participant needs and IDT approval.
Interacts with the public and other employees in a positive manner
Demonstrate an ability to treat a wide range of geriatric diagnoses and be knowledgeable in therapy treatments, goals and contraindications for treatments. Treatments are adapted to the unique PACE population. Requests consultation, supervision or continuing education when appropriate to learn specialized techniques or treatment approaches.
Provide participant/caregiver education and counseling regarding issues related to rehabilitation treatment and goals, safety risks and equipment needs.
Document all PT evaluations and interventions in appropriate agency format and within agency timelines. Document reflect good clinical documentation.
Responsible for instruction and supervision of PTA’s and PT Aides in accordance with PT WACs. Supervises students as assigned.
Work independently managing own caseload and work schedule including PT treatments, evaluations, staff and caregiver training, home visits, required meetings and documentation.
Maintain all participant information and records in a professional and confidential manner, per HIPAA regulations.
Required qualifications for this position includes:
Bachelor's Degree in Physical Therapy
Licensed or eligible for PT licensure in the state of Oregon.
American Heart Association Basic Life Support (AHA-BLS) for Health Care Provider certification required at time of hire.
Valid Driver's License
Current auto liability insurance
Preferred qualifications for this position includes:
Master's Degree or D.P.T in Physical Therapy
One (1) year Working with a geriatric population
