Providence is calling a Clinical Pharmacist Specialist - Ambulatory Care (1.0 FTE, Days) to work at Providence Milwaukie Hospital located in Milwaukie, OR. This position will also float to Providence Portland Medical Center and Providence Willamette Falls Medical Center. You will typically split 2 days per week in 1 hospital and 3 days per week in another hospital.
Hours for this position can start as early as 7am and end as late as 6pm. Shift will be 5 8-hour shifts M-F.

In this position you will have the following responsibilities:
The Clinical Pharmacy Specialist (CPS) for Anticoagulation is responsible for providing and coordinating pharmaceutical care services and collaborative drug therapy management for patients referred to the Providence Health System's Anticoagulation Clinic. In addition, the CPS will serve as a resource to the pharmacy, nursing and medical staffs within the field of anticoagulation.
Ability to effectively evaluate and adjust patient specific anticoagulation therapy.
Ability to communicate effectively with patient's of varying social, educational and cultural backgrounds.
Ability to prioritize multiple responsibilities in an ever growing, busy clinic.
Ability to effectively develop an ongoing relationship with the clinic patients in order to assure optimum outcomes.
Ability to effectively and completely document all patient and physician communications and interactions.
Acceptance of responsibility for patient anticoagulation therapy outcomes.

In this position you will:
• Provides collaborative drug therapy management to patients referred to the anticoagulation clinic to include:
• Initiation of warfarin.
• Warfarin dosage adjustment.
• Protime monitoring using the Coaguchek Plus protime monitor.
• Compliance monitoring.
• Patient and family education.
• Drug/drug interaction monitoring.
• Drug/food interaction monitoring.
• Follow-up instructions.
• Coordination with home health services as applicable in order to assure optimum patient outcomes.
• Effectively communicates and documents all activities occurring in the anticoagulation clinic records to include:
• Anticoagulation Plan of Care.
• Anticoagulation Progress Notes.
• Communication Notes/Special Instructions.
• Laboratory Quality Assurance Records.
• Anticoagulation Teaching Records.
• Participates in all required training and proficiency testing for the laboratories point of care testing program.
• Coordinates ongoing quality assessment and improvement activities for the anticoagulation clinic. Provides quarterly assurance reports to the P&T Committee.
• Serves as a resource to the pharmacy, nursing and medical staffs in the area of anticoagulation.
Required qualifications this position include:
Graduate of an accredited college of pharmacy.
Current valid pharmacy license by the Oregon Board of Pharmacy.
Preferred qualifications for this position include:
PharmD or Master's Degree
Completion of PGY1
Experience in the area of anticoagulation dosing and monitoring.
2 years of experience in hospital or ambulatory care pharmacy
