We are seeking an Associate Social Services - Beaverton Social Services who will collaborate with the primary social workers to meet the social, emotional, financial and care needs of the participant. Provision of social services occurs within day health center environment and occasionally the home environment. Assist the primary social worker with case management aspects of the participants care needs
In this position you will have the following responsibilities:

Demonstrate competency by knowing and applying the principles of an inclusive work environment.participant with financial issues such as Medicare acquisition- representative payee process, ensuring room & board payment, problem-solving PIF issues. Funeral planning. Assist with paperwork relating to Medicaid eligibility, insurance, personal expenditures, and other services as needed.
Assist with care coordination through scheduling of transition meetings, family conferences.
Assist with crisis intervention, collaborate with MSW for immediate relocation due to emergent situations.
Attend Interdisciplinary /Site SW Team & Social Work Meetings.
Maintain communication of caseload issues with Social Work Supervisor for both mentoring and coverage.
Required qualifications for this position include:
Two (2) plus years of social services experience
One (1) year experience working with a frail or elderly population
