We are seeking a CNA Home Care Coordinator - Elderplace Administration that will be under the direction of the Operations Manager, this position is a direct service provider and has a direct service workload, which is structured to allow for coordination responsibilities. Coordinates the work of the in-home agency aides at initial assessment and ongoing. Assures in-home agency plan allowing participants to remain in a community setting with their ADL and iADL needs being safely met.
In this position you will have the following responsibilities:
Train, coach and monitor in-home caregiver provision of care and services
Assess for Participant caregiver need and type, level of care and capacity to successfully receive in in-home services
Liaison with In-Home Agency representatives to communicate in-home staffing needs and coordinate placement of in home staff in participant homes
Assess and complete initial in-home service plans including task list and other associated forms
Coordinate and documents participant and agency caregiver schedules and tasks. Successfully handover care to ongoing ElderPlace teams at all sites
Provide leadership at site including facilitating interdisciplinary care planning and convening consultations in support of challenging in home or supplemental care needs
Assess participant needs including any change in functioning, identify key issues and communicate to Interdisciplinary Team Members, PEP Leadership or In-Home Agency representatives as appropriate
Be responsible for regular monitoring and reporting of PEP wide utilization of in-home services, hours authorized and review invoice documents prior to authorization for payment
Assure that ElderPlace and Providence Health System policies and procedures are followed
Assure adequate staffing
Identify and communicate education and training needs to PEP leadership and In-Home Agency representatives
Be responsible for monitoring and surfacing any issues related to compliance with established policies
Monitor contracted in-home caregivers and review in-home records for quality and adherence to in-home policy
Required qualifications for this position includes:
Coursework/Training: Completion of approved medication administration training and/or advanced C.N.A. skills or restorative aide training, and all required in-services
Current Cardiopulmonary resuscitation (CPR) certification
Standard First Aid
Two (2) years of Experience as a caregiver
Preferred qualifications for this position includes:
CNA2 with current license to practice in the State of Oregon
One (1) year of experience with case management or coordinating long term care services for elderly and people with disabilities.
