Providence Health & Services Behavioral Health Case Manager in Beaverton, Oregon

Description:

Providence St. Joseph Health is calling a Behavioral Health Case Manager to our location in Beaverton, OR.

In this position you will have the following responsibilities:

  • Uses a systematic process and clinical skills to assess, plan, implement and evaluate an individual plan of care for a patient that spans the continuum of care. The plan of care is developed in conjunction with the Primary Care Provider, individualized care team, the member and the member’s family or caregiver(s).

  • Uses principles of utilization management in the decision-making process to assure both quality and cost-effectiveness.

  • Considers alternatives to requested care. Acts as a patient advocate to assure that needed health services are provided in a timely manner. Identifies, researches, and/or reports potential quality of care issues.

  • Monitors and reassesses the plan of care to ensure that quality, quantity and goal achievement are appropriate and cost-effective.

  • Demonstrates positive interactions with patients, physicians and staff so that productivity and coordination of services are maximized.

  • Actively supports continuous quality improvement and population disease management activities. Participates in clinical protocol and guideline development.

Qualifications:

Required qualifications for this position include:

  • Master of Social Work degree from a school of social work accredited by the Council on Social Work education.

  • Minimum of 3 - 5 years of clinical experience.

  • Minimum one year managed care experience and/or utilization management.

  • Demonstrated ability to work cooperatively within a multi-disciplinary team.

  • Demonstrated knowledge of confidentiality, health information privacy and protection rules; ability to maintain knowledge and compliance with regulatory requirements.

  • Demonstrated ability to effectively assist patients, families and support persons in finding satisfactory solutions or adjustments to medical, social, emotional and financial problems. Takes into account an individual's physical, cognitive, emotional, social, ethnic, cultural, religious, gender and age-related needs.

  • Demonstrated knowledge of nursing process/clinical problem-solving process and disease management principles.

  • Demonstrated wide base of sound clinical skills with emphasis on advanced knowledge of care planning and implementation.

  • Demonstrated working knowledge of payer systems and available community resources.

  • Demonstrated ability to work autonomously with little direction and be accountable for practice, including time management, verbal and written communication skills, listening skills, problem solving, decision-making, priority setting, work delegation and work organization.

  • Effective in facilitating group consensus in treatment planning and is skilled in collaboration, consultation, conflict resolution, negotiation and team building.

  • Current Oregon or Washington driver’s license and reliable personal transportation.

Preferred qualifications for this position include:

  • Experience in home health, skilled nursing facility and/or community health nursing (OR) clinical social work.

  • Licensed Clinical Social Worker (LCSW).
