Providence Health & Services Outreach Specialist - Emergency Services in Portland, Oregon
Providence St. Vincent Medical Center's Emergency Services will be looking for an Outreach Specialist. This future opening will be for a full time, non-exempt position.
This posting is not a specific job vacancy. This posting serves to establish interested candidates for future opportunities as an Outreach Specialist for our Emergency Department at Providence St. Vincent Medical Center.
In this position, you will:
Develop and implement strategies to improve the health of individuals who are identified with high utilization of emergency department services. These individuals often lack access and/or knowledge of a connection to primary care as a means to address chronic or non-acute conditions.
Work in the community and coordinate care to help bridge the gaps and eliminate barriers to health care and social services.
Conduct home and site visits to assess risks, match individuals to identified/needed services, and empower individuals to access community resources through referrals, education, and outreach.
Recognize potentially serious problems to prevent poor health and social outcomes for individuals and communities in collaboration with the individual's healthcare team.
Required qualifications for this position include:
Master’s degree in Social Work from an School of Social Work that is accredited or in candidacy status by the Council of Social Work Education (CSWE) -or- Master's degree eligible for licensure as Licensed Professional Counselor (LPC) or Licensed Marriage and Family Therapist (LMFT) in accordance with the Oregon Board of Licensed Professional Counselors
Licensed Clinical Social Worker (LCSW) -or- LPC -or- LMFT licensure with the state of Oregon required.
2 years post masters experience in a behavioral health setting.
A Certified Alcohol and Drug Counselor (CADC) -or- 60 hours of approved continuing education units (CEUs) in diagnosis and treatment of substance use disorders within 1 year of hire
American Heart Association (AHA) Basic Life Support for Health Care Provider completion within 1 month of hire
Valid driver’s license and reliable transportation to homes of patients as needed
In compliance with company policy and state regulations, a Department of Human Services (DHS) criminal background check may be required for this position based upon the location of the position. Fingerprinting may be required also.
Preferred qualifications for this position include:
- Additional experience in substance use treatment setting
