Providence Health & Services Mental Health Registered Nurse - Home Health (0.8 FTE, Days) in Portland, Oregon

Providence Home Health is calling a Mental Health Registered Nurse to visit patients in the West Portland area.

This position is full-time and benefit eligible.

In this position, you will:

  • Provide quality care to patients in their homes under the direction of a physician.

  • Function as a consultant in the nursing assessment of patients who are managed by other disciplines in the department and coordinates patient care activities with other members of the health care team.

  • Be responsible for the assessment, treatment, and care of patients of all ages including adults and geriatric.

  • Individualize patients' treatments and reflect an understanding of the developmental needs and range of treatment requirements.

  • Be responsible for monitoring medical, physical and psychiatric care needs of assigned patients in Home Health.

Qualifications:

Required qualifications for this position include:

  • Current unencumbered Oregon Registered Nurse (RN) License

  • Bachelor’s degree in nursing from an accredited school of nursing

  • Current Basic Life Support (BLS) for Healthcare Provider or Advanced Cardiovascular Life Support (ACLS) certification within 30 days of hire

  • 3-5 years of experience in acute care or community mental health setting

  • Valid driver’s license and reliable transportation

  • Driving record consistent with GOP Safe Driving Policy 330 Incident Point Matrix at time of hire

  • Experience using electronic medical record software

  • In compliance with the Providence Health & Services-OR Human Resources policy #302 “Background Checks”, a criminal background check is required for this position, based on assigned duties.

Preferred qualifications for this position include:

  • American Nurses Credentialing Center (ANCC) Psychiatric Certification

  • Community mental health experience and geriatric experience
